(WMC-TV) - The Department of Veteran's Affairs Office of Inspector General made a report public Wednesday that said the Memphis VA Medical Center staff was responsible for three ER patients' death.

Government sources tell Action News 5, Former Memphis VAMC Director, James Robinson III who is now CEO of Methodist South Hospital, joined a long list of Department of Veterans' Affairs health network employees called out for poor management.

In 2011 they all received big bonuses. That is despite patient deaths and reports of mismanagement, unsanitary conditions and unprofessional practices.

The report shows Robinson received a VA bonus of $10,782. Overall the report shows top VA executives and medical professionals who received more than $3.3 million in bonuses, which are supposed to reward exceptional work.

Multiple Veterans' Affairs Inspector General reports show wide spread problems with mismanagement with VA facilities and VA bonus pays with no clear link to performance.

The OIG issued its report after three veterans died while in the care of the Memphis VA Medical Center. They deemed the hospital's response to the complaints as inadequate.

Representative Jeff Miller, Chairman, House Committee on Veterans' Affairs had the below to say about the recent findings of neglect:

"Until VA leaders make a serious attempt to address the department's widespread and systemic lack of accountability, I fear we'll only see more of these lapses in care."

Robinson did not return Action News 5's calls for comment.
